Membership to the FIVP is conditional on the practice being independently owned.* FIVP reserve the right to request further information on practice ownership if required. FIVP logo must only be used with prior written agreement from the FIVP. The annual membership fee is non-refundable once membership has been confirmed. The practice must be able to demonstrate that it meets Core Standards**
